Records Release Authorizations

Federal regulations (49 CFR Part 391) require your written authorization for us to verify your driving and safety records. Please review and authorize each item below.

1. Motor Vehicle Record (MVR). I authorize , and any agent or consumer reporting agency acting on its behalf, to obtain my motor vehicle record from any state driver-licensing agency in which I hold or have held a license, both before hire and at least once every twelve months during employment (49 CFR 391.25). I acknowledge this release is authorized under the Driver's Privacy Protection Act (18 U.S.C. § 2721).

2. Safety Performance History. I authorize to investigate my employment and safety performance history, and I authorize my previous employers to release that information (including accidents under 49 CFR 390.5 and drug/alcohol testing violations under 49 CFR Part 40) for the preceding three years (49 CFR 391.23).
